CPT code 99205 is the highest-level E/M code used for an office or other outpatient visit for a new patient. Because it sits at the top of the new patient range, it's also one of the codes payers review most closely. 99205 CPT Code Description for New Patient Office Visits
The 99205 CPT code describes an office or outpatient visit for a new patient that requires a medically appropriate history or exam and a high level of medical decision making (MDM).
Since 2021, you can select 99205 based on either high-complexity MDM or total time, and you only need one of the two.
Who Counts as a New Patient for 99205
A new patient is someone who has not received professional services from the provider, or from another provider of the exact same specialty and subspecialty in the same group, within the past three years. If they have, the visit belongs in the established patient range, from 99211 to 99215.
99205 Time Requirement
When you bill 99205 by time, the requirement is at least 60 minutes of total provider time spent on the date of the encounter. If you bill based on MDM, you don't need to document time at all.
What Counts Toward Total Time
Total time includes face-to-face time with the patient and related indirect tasks on the same day, like reviewing medical records, ordering tests, counseling, coordinating care, and writing documentation. Time spent by clinical staff doesn't count; only the physician's or other qualified health care professional's time does.
99205 Time Range: What Changed in 2024
For years, 99205 was described with a time range of 60 to 74 minutes, and many references still use it. Since 2024, CPT describes office visit times as minimums instead, so the 99205 time requirement is now 60 minutes that must be met or exceeded.
Billing Prolonged Time Beyond 99205
Extra time is reported with an add-on code, and the threshold depends on the payer:
- CPT 99417: used by payers that follow CPT, starting at 75 minutes of total time, with one unit for each additional 15 minutes.
- HCPCS G2212: used for Medicare, starting at 89 minutes, as set out in CMS's Evaluation and Management Services booklet.
Both can only be added when 99205 was selected based on time.
High-Complexity MDM Requirements for 99205
Under the AMA E/M guidelines, 99205 requires high complexity in at least two of the three MDM elements:
| MDM element | What high complexity requires |
|---|---|
| Problems addressed | One or more chronic illnesses with severe exacerbation, progression, or side effects of treatment, or an acute or chronic illness or injury that poses a threat to life or bodily function |
| Data reviewed and analyzed | At least two of three categories: a combination of three items (external notes, test results, tests ordered, or an independent historian), independent interpretation of a test performed by another provider, or discussion of management with an external provider |
| Risk of patient management | High risk, such as drug therapy requiring intensive monitoring for toxicity, decisions about major surgery or hospitalization, or a decision to de-escalate care because of poor prognosis |
Meeting only one element at high complexity isn't enough, and that visit belongs to 99204.
99204 vs 99205: Key Differences
| CPT 99204 | CPT 99205 | |
|---|---|---|
| MDM level | Moderate | High |
| Time (minimum) | 45 minutes | 60 minutes |
| Problems addressed | Chronic illness with exacerbation or two or more stable chronic illnesses | Severe exacerbation or a threat to life or bodily function |
| Risk | Moderate, including prescription drug management | High, including intensive monitoring, major surgery, or hospitalization decisions |

How 99205 CPT Code Reimbursement Is Calculated
Medicare payment for 99205 comes from three relative value units (work, practice expense, and malpractice), each adjusted by the geographic practice cost index (GPCI) for your locality and multiplied by the annual conversion factor. Commercial payers set their own rates by contract. For the current amount in your area, use the CMS Physician Fee Schedule Look-Up Tool.
If the visit is delivered by telehealth or billed with a procedure on the same day, check the rules for Modifier 95 and Modifier 25.
99205 Documentation Requirements That Hold Up to Review
A 99205 note should make the level clear to anyone who reads it:
- For MDM: name each problem and its severity, describe what data was reviewed and from whom, and state the high-risk decision in the plan.
- For time: record the exact total minutes and the activities they covered.
- For both: confirm new patient status.
Common 99205 Billing Errors
- Billing 99205 when only one MDM element reaches high complexity.
- Writing "reviewed records" without saying which records or what was relevant.
- Documenting time as a range or an estimate, or counting staff time.
- Billing 99205 for a patient seen by the same specialty within three years.
- Coding a visit that clearly meets high MDM as 99204 out of caution.
How Payer Downcoding Policies Affect 99205
Some payers now adjust high-level E/M claims automatically. Under Cigna's R49 policy, in effect since October 1, 2025, claims billed with 99204 and 99205, among other codes, may be reduced by one level when the claim doesn't support the level billed. Clear documentation is what makes an appeal possible.
