The 99211 CPT code is the lowest-level E/M code for established patients, and it's also one that practices often get confused about, either avoiding it entirely or billing it for visits that don't qualify. If you need medical coding or RCM support to get your E/M claims right, talk to our team. If you want to understand the code first, keep reading.

99211 CPT Code Description and When It Applies

The AMA describes CPT code 99211 as an office or other outpatient visit for the evaluation and management of an established patient that may not require the presence of a physician or other qualified health care professional. That last part is what sets it apart from 99212 through 99215.

It applies only to established patients, meaning patients seen by the same physician, or another physician of the exact same specialty and subspecialty in the same group, within the past three years. There's no MDM level and no time threshold to meet, but the visit still needs an evaluation and a management decision.

Why It's Called the Nurse Visit CPT Code

Most 99211 encounters are performed by clinical staff, such as RNs, LPNs, or medical assistants, under a physician's supervision. That's why billing teams often call it the nurse visit CPT code, even though nurses aren't the only ones who can report it.

Is 99211 Still a Valid CPT Code in 2026?

Yes. The 2021 E/M revisions removed the "typically 5 minutes" reference from the descriptor, and some practices read that as the code being retired. It wasn't. CPT code 99211 remains active, and Medicare and commercial payers still reimburse it when the visit qualifies.

Who Can Perform and Bill CPT Code 99211?

Incident-to Requirements for Clinical Staff Visits

When clinical staff perform the visit and it's billed to Medicare under the physician's NPI, it has to meet incident-to rules:

  • The service is part of a plan of care the physician established.
  • The physician performed the initial service and stays actively involved in the patient's care.
  • The physician provides direct supervision, which means being immediately available in the office suite, not necessarily in the room.

Since January 1, 2026, CMS permanently allows virtual direct supervision through real-time audio and video for a subset of incident-to services performed entirely by auxiliary personnel. Audio-only doesn't qualify, so confirm the service falls within that subset before relying on it.

Can Physicians, NPs, and PAs Bill 99211?

Yes. 99211 is commonly called a nurse visit code, but physicians, nurse practitioners, and physician assistants can also report it when the encounter fits. In practice, when a provider performs the visit, it often involves enough evaluation to support 99212 or higher, so it's worth reviewing the note before defaulting to 99211.

Services That Qualify for 99211 and Services That Don't

99211 fits brief, medically necessary visits where clinical staff evaluate the patient and a management decision follows:

  • A blood pressure check that leads to a documented decision to continue or adjust medication
  • A medication monitoring visit where the nurse assesses tolerance and the provider decides whether anything changes
  • A wound check with a documented assessment and next step

It doesn't fit when the service has its own CPT code and nothing more happens. A vaccine or an allergy injection on its own should be billed with its administration code, and the same goes for a routine blood draw. Suture removal is also included in the global period when the same practice placed the sutures.

99211 Documentation Requirements That Support the Claim

The note needs to show more than a vital sign or a procedure:

  • Who performed the service, with their credentials
  • What was evaluated and what was found
  • The management action that resulted
  • Why the visit was medically necessary as part of the patient's care plan
  • That the supervising physician was available, if billed incident-to

A blood pressure reading on its own doesn't support 99211. The same reading with a documented decision to continue the current medication does.

Common 99211 Billing Errors

  • Billing 99211 for a new patient.
  • Billing incident-to when the supervising physician wasn't available.
  • Reporting 99211 for an injection or blood draw with no evaluation.
  • Defaulting to 99211 for provider visits that support 99212 or higher.
  • Stopping 99211 billing altogether because of the 2021 changes.

How 99211 CPT Code Reimbursement Is Calculated

Medicare calculates 99211 payment the same way it does for other E/M codes. It starts with three relative value units (work, practice expense, and malpractice), adjusts each one by the geographic practice cost index (GPCI) for your locality, and multiplies the total by the annual conversion factor.

That's why rates differ by location and between facility and non-facility settings. Commercial payers set their own rates by contract. For the current Medicare amount in your area, use the CMS Physician Fee Schedule Look-Up Tool.

Billing 99211 With Other Services on the Same Day

In general, 99211 shouldn't be billed with another E/M service for the same patient, on the same day, by the same provider. When a procedure with its own code is performed during the visit, such as an injection, the procedure code usually covers the encounter, and 99211 only stands on its own if there's a separately identifiable, documented evaluation.

NCCI edits are updated quarterly, so check the current code pairs before submitting any same-day combination. For when a separate E/M can be reported alongside a procedure, see our guide to Modifier 25.
