What is the 99214 CPT Code?

The 99214 CPT code is defined by the AMA as an office or other outpatient visit for the evaluation and management of an established patient. Under the 2021 E/M guidelines, still active in 2026, code selection for 99214 is driven by one of two independent pathways: Medical Decision Making or Total Time.

A patient qualifies as established when they have received professional services from the same physician or physician group within the same specialty in the past three years. If that threshold is not met, the visit must be coded under the new patient series (99202 to 99205).

What Are the MDM Requirements for CPT Code 99214?

To select 99214 based on MDM, the encounter must reach moderate complexity in at least two of the three MDM elements:

Element 1: Number and Complexity of Problems Addressed

Moderate complexity requires at least one of the following:

  • Management of one or more chronic illnesses with exacerbation, progression, or side effects of treatment
  • Management of two or more stable chronic illnesses
  • Management of one undiagnosed new problem with uncertain prognosis
  • Management of one acute illness with systemic symptoms (fever, vomiting, shortness of breath)
  • Management of one acute complicated injury

Element 2: Amount and Complexity of Data Reviewed and Analyzed

Moderate complexity requires at least one of the following categories, or a combination that meets the threshold:

  • Review and order of each unique lab, radiology test, or other test
  • Review of records from external sources and summarizing the findings
  • Independent interpretation of a test ordered by another provider
  • Discussion with an external physician or appropriate source for management decisions

Element 3: Risk of Complications and Morbidity or Mortality

Moderate risk requires at least one of the following:

  • Prescription drug management
  • Decision for minor surgery without identified patient or procedure risk factors
  • Diagnosis or treatment significantly limited by social determinants of health
  • A new diagnosis requiring intensive diagnostic evaluation

Meeting only one MDM element at moderate complexity does not qualify for 99214. Two of the three must independently reach moderate complexity. If only one element is present, the correct code is 99213.

Time-Based Requirements for CPT Code 99214

When selecting 99214 based on time, the provider must spend a total of 30 to 39 minutes on the date of the encounter. This includes both face-to-face and non-face-to-face time, such as:

  • Reviewing records and results before the visit
  • Ordering tests and medications
  • Counseling and educating the patient
  • Documenting the visit

Staff time does not count. Only physician, NP, or PA time qualifies toward the total.

When billing by time, the note must state the total minutes explicitly. A range is not acceptable. "Approximately 35 minutes" introduces audit risk. "Total time on the date of the encounter: 35 minutes" is the correct documentation format.

2026 Reimbursement Rates for CPT Code 99214

The 2026 Medicare national average reimbursement for CPT 99214 is approximately:

  • Non-facility (office) setting: $121 to $128
  • Facility setting: $88 to $95

These figures reflect the 2026 Medicare Physician Fee Schedule conversion factors. Rates vary by MAC region and geographic practice cost index. Commercial payers typically reimburse at higher rates based on contracted fee schedules.

This is where most coding errors originate. The distinction between 99213 and 99214 comes down to the complexity level required in each MDM element.

The practical test: if the provider is managing a prescription drug, ordering labs with clinical interpretation, or dealing with a condition that carries uncertainty or systemic symptoms, 99214 is almost always the appropriate code. If the visit is a straightforward follow-up with no new decisions and only OTC drug management, 99213 is correct.

How to Document a 99214 Visit Correctly

The most common reason 99214 claims are denied or downcoded is not insufficient care. It is insufficient documentation. The note must actively demonstrate the MDM complexity that justifies the code.

Practical documentation checklist for 99214:

  • Assessment section: Name each problem addressed and describe its current status, active management decisions, or changes to the treatment plan. Listing diagnoses without context does not support 99214.
  • Plan section: Specify the prescription medication being managed, the clinical reason, and any monitoring plan. "Continue current medications" without elaboration does not meet moderate complexity risk.
  • Data section: If labs or imaging were reviewed, document what was reviewed, the clinical significance of the findings, and how those findings influenced the plan.
  • Time documentation: If billing by time, state the total minutes and a brief description of how that time was spent.

One note on the assessment and plan: the complexity of your documentation needs to match the complexity of the code. A three-line plan section does not support a 99214, regardless of how complex the actual encounter was. If you provided moderate complexity care, your note must communicate it.

Common Billing Errors on CPT 99214 Claims

Downcoding out of audit fear. The most expensive mistake. If documentation genuinely supports 99214, billing 99213 costs your practice real money on every qualifying visit.

Treating prescription drug management as automatic. Prescription drug management supports moderate risk, but it must appear in the plan with context. "Refill Lisinopril" is not the same as documenting that you reviewed the patient's blood pressure trend, adjusted the dose, and ordered a follow-up lab.

Combining diagnoses without active management. Listing five chronic conditions in the assessment does not automatically qualify for 99214. Each problem must have a documented management decision tied to the current visit.

Billing 99214 for new patients. The 99214 series is for established patients only. A new patient encounter must use the 99202 to 99205 series.

Ignoring the data element. Many practices focus on problems and risk but neglect to document data review, which is one of the three MDM elements. If you reviewed labs from an outside provider or spoke with a specialist, document it.
